Abstract

Introduction: We have compared the results of intramedullary nailing operations performed for open and closed tibial fractures. Material and
Methods: 94 patients operated for tibial shaft fractures with unreamed intramedullary nailing were included in this study. Fractures were open in 38 and closed in 56 patients. Mean follow up is 26 (9-57) months. Open fractures were type 1 in 19 (%50), type 2 in 13 (%34.2) and type 3 in 6 (%15.8) cases according to Gustilo classification. Static nailing was performed in 86 (%91.5) and dynamic in 8 (%8.5) patients. Follow up results were evaluated according to criteria of Johner and Wrush. Groups were compared with Chi2 test.
Findings: There were no statistically significant difference between open and closed fractures according to Johner and Wrush criteria. Avarage union time was 14.6 (9-36) weeks for closed and 17.8 (11-40) weeks for open fractures. There were no nonunions and no infections causing osteomiyelitis in either groups.
Discussion: We concluded that unreamed intramedullary nailing can be used in type 1 and 2 open fractures of the tibial shaft in acute stage as safely as in closed fractures.
